Bronchiectasis

Bronchiectasis means abnormal dilatation of the bronchi.

Now the causes of bronchiectasis:

#Congenital:
cystic fibrosis
primary ciliary dyskinesia
kartagener's syndrome
(sinusitis & transposition of the viscera)

#Acquired(children):
whooping cough, measles
primary tuberculosis

#Acquired(adult):
pneumonia
pulmonary TB
allergic bronchopulmonary aspergillosis

Clinical features:

Chronic cough
purulent sputum
pleuritic chest pain
haemoptysis
halitosis
fever, anorexia, malaise, weight loss


Investigation:

CBC
Sputum culture
Chest x-ray: Collapse/Consolideration may be seen
CT Scan of chest is more sensetive

Management:
#Chest physiotherapy:
   Patients should lie in position,
   take deep breath and then
   forcefully expired the air;
   which helps the secretion move
   from bronchi to trachea!
   then vigorous coughing !
   this done for 5-10 minutes
   twice a day.

#Antibiotics therapy:
   Oral ciprofloxacin 500 mg twice daily

#Surgical treatment:
   Excision of bronchiectatic area.
